To determine the relationship between clinical findings of patients with head trauma and skull fracture by x-ray and CT scan in pediatric patients, areview of 73 admitted patients with ages 0-21 years old who had both skull roentgenography and CT scan of the head for head trauma over a 31 year period was done. There was an incidence of skull fracture of 23.3% with 1.4% mortality. To make efficient use of resources and minimize exposure to radiation, clinical criteria are necessary to select patients for both procedures. Other criteria previously defined and modified by Phillips are not significant in our series. On the basis of the present study, 3 criteria are identified as significant to consider the possibility of having a skull fracture after head trauma. These are hemotympanum, history of possible skull penetration and vomiting. The study also found the probability of hematoma occurring in the presence of skull fracture. We therefore conclude that patients with head trauma should undergo radiologic evaluation if they present with the modified criteria by Phillips and consider also vomiting as an important indicator of the possibility of the presence of skull fracture
