This is a case of an 8-year old female presenting with swelling of the left upper lid and a mass in the superolateral area of the left orbit on computed tomography. The initial impression was orbital inflammatory disease. Oral steroid therapy was started but there was absence of resolution of orbital signs and symptoms. Excision biopsy of orbital mass with frozen section was done. Histopathologic diagnosis was chronic inflammatory process. Three months postoperatively, there was recurrence of swelling of the left upper lid. Repeat CT scan cannot rule out recurrence. Review of the histopathologic specimen was done for which the diagnosis of chronic inflammatory disease was re-affirmed. The patient was referred back to Pediatric Service where steroid therapy restarted. Orbital inflammatory signs were noted to have improved favorably to the treatment. The patient is currently being managed by both Ophthalmology and Pediatric Service as orbital inflammatory disease
